About the Role

We are seeking an experienced Mental Health Nurse to join our County LMHT teams in Gedling. The successful candidate will have the opportunity to develop skills needed to work within a community setting, providing care to an adult population across the Nottingham city area, open to secondary care services, within SMI pathways (mood affective, complex psychosis, community rehabilitation and non psychosis).

Key Responsibilities

  • Hold a caseload and work with patients and their families to support personalised care plans and provide specialist interventions.
  • Promote the recovery of patients and through working with partner agencies and primary care to ensure timely and safe discharge from secondary care.
  • Complete both triage assessments and more detailed initial assessments of new referrals.
  • Provide support via the teams duty system, either from telephone support or face to face.

Requirements

  • Professional Qualification (Mental Health) to degree level or equivalent.
  • Maintains current registration with the NMC.
  • Further specialist knowledge/post registration experience in specific service area or other formal training.
