Service Manager for Hospital Discharge Operations

About the Role
We are seeking an experienced leader to manage and deliver our Hospital Discharge Support Service. This is a full-time opportunity working with Age UK Buckinghamshire to ensure smooth transitions for patients leaving hospital after a stay.
The post holder will be responsible for overseeing the day-to-day operation of the service, supporting staff and volunteers from our offices in Aylesbury. With a focus on delivering high-quality care, you will work closely with our Community Team to provide practical support to residents settling into their homes.
In a typical year, our teams help over 1,000 patients return home safely, offering crucial assistance to people aged 18 and over who require additional support following hospital discharge.
