About the Role

Acorns Children's Hospice is seeking a Family Practitioner to provide specialist psychosocial support to families in Birmingham. As a member of our Family Services Team, you will work with families who are experiencing bereavement and provide holistic support to help them cope with their loss.

Key Responsibilities
  • Provide ongoing psychosocial support to families at home, hospice, or other appropriate environments.
  • Conduct initial holistic needs assessments to identify the needs of each family.
  • Develop and implement therapeutic plans to support families in their grieving process.
  • Collaborate with other professionals to ensure an integrated approach to supporting families.
  • Maintain professional development and registration.
About You

We are looking for a qualified professional with experience in family-centred social care or nursing in the community. You should have a degree in a relevant field and be registered with a professional body. You will also have experience of managing caseloads in a complex multi-agency environment and have up-to-date child protection knowledge.
