About the Role

At Acorns Children's Hospice Trust, we are seeking a highly skilled and compassionate Family Practitioner to join our Family Services Team. As a Family Practitioner, you will play a vital role in providing specialist psychosocial support to families who are facing the challenges of a child's life-limiting condition.

Key Responsibilities

  • Provide ongoing holistic psychosocial support to families, including bereavement and end-of-life care.
  • Conduct initial holistic needs assessments to identify the family's needs and develop a personalized support plan.
  • Deliver a range of face-to-face therapeutic interventions, including group work, to support families in coping with their child's condition.
  • Coordinate additional support for parents, siblings, and relatives as they navigate the grieving process.
  • Work collaboratively with other professionals to ensure an integrated approach to care.
  • Maintain your own professional development and registration.

About You

We are looking for a candidate with a strong background in family-centred social care or nursing, with experience of working with children with life-limiting conditions. You must be professionally registered with a degree and have experience of managing caseloads in a complex multi-agency environment.
