About the Role
At Acorns Children's Hospice Trust, we are seeking a highly skilled and compassionate Family Practitioner to join our Family Services Team. As a Family Practitioner, you will play a vital role in providing specialist psychosocial support to families who are facing the challenges of a child's life-limiting condition.
Key Responsibilities
- Provide ongoing holistic psychosocial support to families, including bereavement and end-of-life care.
- Conduct initial holistic needs assessments to identify the family's needs and develop a support plan.
- Deliver a range of face-to-face therapeutic interventions, including group work, to support families in coping with their child's condition.
- Coordinate additional support for parents, siblings, and relatives as they navigate the grieving process.
- Work collaboratively with other professionals to ensure an integrated approach to care.
- Maintain your own professional development and registration.
About You
We are looking for an experienced and registered professional who has a background in family-centred social care or nursing. You will have a broad, varied background or experience working in a community setting, and be able to travel independently across the region. You must be driven by children's needs and have the emotional resilience to support families going through loss and bereavement.
