Social Care Liaison

Job Description
The Social Care Liaison will be responsible for providing high-quality support and guidance to individuals living with mental health and dementia conditions. This will involve working closely with multidisciplinary teams to ensure the delivery of excellent care and support services.
Key Responsibilities
- To provide individualised support and guidance to individuals living with mental health and dementia conditions.
- To work collaboratively with multidisciplinary teams to ensure seamless care and support services.
- To maintain accurate records and reports on individual progress and outcomes.
Requirements
- A minimum of 2 years' experience working in a similar role within the healthcare sector.
- Proven ability to work effectively in a fast-paced environment and manage competing priorities.
- Degree-level qualification in a relevant field or equivalent experience.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and colleagues.
