We are seeking a highly skilled and experienced Mental Health Nurse to join our Crisis Resolution Home Treatment Team as a Crisis Resolution Home Treatment Nurse. As a key member of the team, you will play a vital role in providing high-quality, patient-centered care to individuals experiencing mental health crises.
Key Responsibilities- Provide urgent and emergency triage, advice, and assessment for individuals in mental health crisis.
- Undertake home treatment visits as indicated by the Multi-Disciplinary Team (MDT).
- Deliver highly skilled nursing care, including care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.
- Work collaboratively with the crisis house to lead on care for admissions as a service and facilitate early discharge from inpatient wards where clinically indicated.
- Provide clinical leadership and supervision for junior staff.
