Main Job Responsibilities

We are seeking an experienced and skilled Adult Disability Senior Practitioner to join our team at NHS Dorset. As part of our Personal Health Commissioning Team, you will be responsible for managing a caseload of CHC eligible individuals in accordance with the National Framework for Continuing Healthcare.

Your key responsibilities will include:

  • Managing a caseload of complex cases, undertaking annual eligibility reviews, and managing Continuing Healthcare packages of care.
  • Ensuring value for money placements that meet the needs of the individual and identifying opportunities for development and improvement to assist in the delivery of best practice healthcare.
  • Collaborating with a range of healthcare professionals, community, and acute services to ensure high-quality assessments and timely service delivery.
  • Working flexibly and autonomously, with the ability to work collaboratively with Health and Social Care practitioners, patients, families, and carers.

We also offer a hybrid model of working, with a percentage of contracted hours being on-site and the remainder being home-based.

Person Specification

We are looking for an individual with:

  • Extensive knowledge of specialist areas acquired through experience and training, plus further specialist knowledge of the Continuing Healthcare process.
  • Good understanding of Personal Health Commissioning.
  • Experience of working in a health environment, with negotiation and conflict management skills and the ability to influence.
  • Previous experience of caseload management.
Skills and Abilities

We are seeking an individual with:

  • Demonstrated capability to act upon incomplete information, using experience to make inferences and decision-making.
  • Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales.
  • Ability to analyze very complex cases where material is conflicting and drawn from multiple sources.
  • Effective communication skills and evidence of continued professional development.
  • Ability to deliver professional standards in all recording and assessment reports.
Qualifications

We require an individual with:

  • Educated to degree level in a relevant subject or equivalent level of experience working at a similar level in a specialist area, ideally Personal Health Commissioning.
  • Nursing qualification and registered with a professional body.
  • Knowledge of NHS Personal Health Commissioning and associated procedures and practices, underpinned by theoretical knowledge of the National Framework for NHS Personal Health Commissioning and NHS-funded Nursing Care and Children's and Young People's Continuing Care.
  • Evidence of Continued Professional Development.
