Job Summary

The Royal Devon and Exeter NHS Foundation Trust is seeking a highly skilled and motivated Clinical Lead to join our Hospital Discharge Team. As a key member of the team, you will play a crucial role in managing and supporting healthcare clinicians in complex discharge planning.

About the Role

This is an exciting opportunity to work in a dynamic and supportive multidisciplinary team, fostering good relationships with care home providers and supporting smooth transitions from hospital to care home environments. You will be responsible for using a patient-centric approach to complete nursing assessments and formulate discharge plans.

Key Responsibilities
  • Manage and support healthcare clinicians in complex discharge planning
  • Foster good relationships with care home providers
  • Support smooth transitions from hospital to care home environments
  • Use a patient-centric approach to complete nursing assessments and formulate discharge plans
  • Work collaboratively with Specialist Nurses and Physiotherapists to expand knowledge in multidisciplinary complex care
