Nurse for Older Adults

This is an excellent opportunity for an experienced nurse to join our innovative Early Intervention Team, working proactively to prevent unnecessary admissions and promote patient-centered care. The role involves supporting patients and their families by comprehensive screening and assessment, as well as preventing unnecessary admissions into acute care or initiating early discharge planning.
The post holder will be responsible for providing a high standard of assessment, advice, and evaluation of patients to formulate ongoing plans of care. This includes developing specialist knowledge of ED, Frailty, and Discharge Planning specific assessments, with opportunities to evaluate practice and audit outcomes.
What We Offer- A competitive salary of approximately £41,000-£47,000 per annum.
