Key Responsibilities- Provide personal care and support to individuals with complex needs, including meal preparation and administration when necessary.
- Develop and maintain strong relationships with the individuals we support, their families, and other stakeholders to ensure person-centered care.
- Safeguard and promote the welfare of the individuals we support, adhering to our organization's values and policies.
- Support individuals to make choices and express their individuality, promoting their autonomy and dignity.
- Work collaboratively with other agencies, parents, carers, and services to ensure comprehensive care and support.
- Take direction from the multi-disciplinary team and contribute to the development of care plans and support strategies.
- Ensure good working practices, including health and safety awareness and adherence to organizational policies.
- Assist with tasks necessary to ensure positive life experiences for the individuals we support.
- Develop and maintain accurate records and support plans, and function as a key worker when required.
- Support individuals to maintain a clean and safe living environment, tailored to their individual needs.
- Participate in regular and ongoing training to enhance your skills and knowledge, aligned with our organization's values and goals.
- Experience of supporting individuals with complex health needs, including sight loss and life-limiting degenerative conditions.
- Ability to work flexibly on a shift rota basis, including evenings, weekends, and bank holidays.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and stakeholders.
- Compassion, empathy, and a commitment to person-centered care.
- Ability to work as part of a team, with a flexible and adaptable approach.
- Basic knowledge of health and safety procedures and organizational policies.
