Job Title: Band 6 Community Mental Health Practitioner

We are seeking a highly skilled and dedicated Band 6 Community Mental Health Practitioner to join our dynamic and friendly Integrated Mental Health Team based in Cannock.

About the Role

The Integrated Mental Health Team (IMHT) is an innovative secondary care community team that provides evidence-based psychological and medical interventions to patients with complex mental health needs. As a Band 6 Community Mental Health Practitioner, you will be responsible for managing a complex caseload, including specialist assessment, care planning, and evaluation of individual treatment plans.

Key Responsibilities
  • Participate in the setting of quality standards, including auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
  • Administer medication, complying with relevant Trust procedures and NMC Guidance.
  • Assess patient's individual care needs, develop, implement, and evaluate programmes of care.
  • Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance.
  • Complete and contribute to robust risk assessments and risk management plans.
  • Provide leadership with the MDT, support junior colleagues in their development, and facilitate student nurses in their training.
