Job Overview

We are seeking a highly skilled and dedicated Band 6 Community Mental Health Practitioner to join our Integrated Mental Health Team in Cannock. As a key member of the team, you will be responsible for providing high-quality care and support to individuals with complex mental health needs.

The Integrated Mental Health Team is an innovative secondary care community team that provides evidence-based psychological and medical interventions to patients with a wide range of mental health needs. Our team is guided by collective leadership and is committed to delivering high-quality care and support to our service users.

As a Band 6 Community Mental Health Practitioner, you will be responsible for managing a complex caseload, which includes specialist assessment, care planning, and evaluation of individual treatment plans. You will also be expected to participate in the setting of quality standards, administer medication, and deliver a range of interventions in accordance with the agreed plan of care.

Main Duties of the Job
  • Manage a complex caseload, including specialist assessment, care planning, and evaluation of individual treatment plans.
  • Participate in the setting of quality standards, including auditing, monitoring, and reviewing of practice in line with current clinical guidance and policy.
  • Administer medication, complying with relevant Trust procedures and NMC Guidance.
  • Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance.
  • Complete and contribute to robust risk assessments and risk management plans.
