About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our Community Team. As a Band 6 Community Mental Health Practitioner, you will be responsible for managing a complex caseload, assessing patient needs, and developing individualized care plans.

Key Responsibilities
  • Participate in quality standard setting, auditing, and reviewing practice in line with current clinical guidance and policy.
  • Administer medication, adhering to Trust procedures and NMC Guidance.
  • Assess patient care needs, develop, implement, and evaluate care programs.
  • Deliver interventions in accordance with agreed care plans and Pathway guidance.
  • Contribute to robust risk assessments and risk management plans, promoting positive risk-taking to improve quality of life.
  • Provide leadership, support junior colleagues, and facilitate student nurses in their training.

Person Specification
  • Recorded/registered qualification in RMN, Social Work, or Occupational Therapy.
  • Active on NMC/HCPC Register.
  • Experience working with clients presenting with emotional dysregulation, high-risk behavior, and complex needs.
  • Experience working in community settings and making autonomous decisions.
