**Job Title:** Care Specialist

We are seeking an experienced Care Specialist to join our team at Mountbatten Hampshire.

**About the Role:

The successful candidate will work with a multi-professional team to provide social work support to inpatient units, community teams, and day services. They will identify patient goals and tailor care to meet these goals, acting as a safeguarding lead and providing regular training to ensure a high level of data quality.

Responsibilities include maintaining records and statistical data, assessing and managing risk to individuals and communities, working with diverse patients, carers, and families, and providing expert opinions on the Care Act and Social Care. The role also involves negotiating with local services to meet patient needs, carrying out assessments and discharge planning.

The ideal candidate will stay up to date with training and professional registration, providing learning opportunities for student social workers and participating in service improvement projects. Effective communication with patients, carers, and colleagues is essential, adhering to policies and procedures and demonstrating a commitment to equality and diversity.

We offer a competitive salary of £35,000 - £40,000 per annum, depending on experience, and a range of benefits including a contributory pension scheme and annual leave entitlement.
