About the Role

We are seeking an experienced Healthcare Navigator to join our team at Community Vision.

In this role, you will work closely with parents, carers, and professionals in health and social care to ensure that children and young people with Down syndrome receive the best possible healthcare services.

Your key responsibilities will include:

  • Navigating the health and care system to identify areas for improvement;
  • Developing and implementing strategies to improve health outcomes for children and young people with Down syndrome;
  • Collaborating with our team and stakeholders to ensure effective communication and collaboration;
  • Providing education and support to parents and carers to empower them to navigate the health and care system.

Requirements

To succeed in this role, you will need to be a Qualified Learning Disability Nurse or other registered Health Professional with a passion for improving healthcare services for individuals with Down syndrome.

What We Offer

We offer a competitive salary range of $60,000 - $80,000 per annum, depending on your experience and qualifications.
