About the Role
We are seeking an enthusiastic and organised Team Leader to join our professional Health Visiting team at Kent Community Health NHS Foundation Trust. As a key member of our team, you will play a vital role in supporting the delivery of high-quality patient care and contributing to the excellent experience our patients and service users have every day.
Key Responsibilities
- Provide leadership and guidance to the Health Visiting team, ensuring that they have the necessary support and resources to deliver excellent patient care.
- Work closely with clinical teams to develop and implement effective care plans that meet the needs of our patients.
- Contribute to the development and implementation of policies and procedures that promote excellent patient care and safety.
- Collaborate with other teams and stakeholders to ensure that our patients receive seamless and coordinated care.
- Monitor and evaluate the effectiveness of care plans and make recommendations for improvement.
Requirements
- Proven experience of working in a healthcare setting, preferably in a leadership or management role.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
