Lancaster Medical Practice is committed to providing high-quality care and support to vulnerable adults in our community. We are seeking an experienced Adult Mental Health Practitioner to join our team and help us achieve this goal.

As a key member of our team, you will be responsible for responding to distressed patients in a non-threatening manner, building relationships with patients to encourage trust, and identifying potential risks of harm to themselves or others.

In this role, you will work closely with patients, their families, and other healthcare professionals to develop and implement effective care plans. You will also be responsible for maintaining accurate patient records, liaising with professionals in the community treatment team, and establishing good networks with partner organisations.

**Key Responsibilities:**

  • Respond to distressed patients in a non-threatening manner
  • Build relationships with patients to encourage trust
  • Maintain patient records and produce care plans and risk assessments
  • Liaise with professionals in the community treatment team
  • Establish good networks with partner organisations

**Requirements:**

  • Degree in a relevant field (e.g., psychology, social work)
  • Experience working with vulnerable adults
  • Strong communication and interpersonal skills
  • Ability to work autonomously and as part of a team

**Salary:** £38,000 - £48,000 per annum depending on experience
