Job Summary

We are looking for a skilled Care Coordinator to join our team at TWNS PCN. As a key member of our Health and Wellbeing Team, you will be responsible for coordinating care plans and ensuring seamless transitions between care settings.

Main Duties:
  1. Collect data on patients/carers for recognised outcome measures and document for service interpretation.
  2. Contribute to audits and data collection to aid evaluations of the PCN services.
  3. Advise patients on diet, lifestyle, physical and mental wellbeing, and signposting to local services and funding they may be eligible to access.

The salary for this position is estimated to be around £28,000 - £35,000 per annum, depending on experience. If you are a motivated and organized individual with excellent communication skills, we encourage you to apply for this exciting opportunity.
