Key Responsibilities:
  • Carry out daily well-being checks and respond to emerging changes in circumstances.
  • Identify and manage risk, including safeguarding, and take accountability for ensuring that appropriate actions are taken.
  • Update customer records in relation to well-being and risk, and manage workloads effectively.
  • Engage with older people and their families to identify issues and support them in progressing their concerns.
  • Develop relationships with partner agencies to provide well-being opportunities and support customers to access these services.
  • Routinely carry out health and safety, fire safety, and warden call system checks to ensure compliance and high service standards.
Requirements:
  • A strong commitment to delivering high-quality customer service in a fast-paced environment.
  • Natural relationship-building skills, with the ability to motivate and inspire others.
  • Experience of partnership working with specialist agencies for older people, with a passion for co-producing innovative services.
  • Demonstrable knowledge of Adult Safeguarding.
  • Excellent interpersonal skills, with the ability to adapt your approach depending on your audience.
  • Comprehensive understanding of professional boundaries and high personal integrity.
  • Good general IT skills, including Microsoft Excel and in-house systems.
