About the Role

We are seeking a highly skilled and experienced Clinical Triage and Rehabilitation Specialist to join our team at a prestigious private hospital in Leatherhead. As a key member of our multidisciplinary team, you will play a vital role in delivering top-tier rehabilitation services to patients with neurological and complex conditions.

Key Responsibilities
  • Conduct thorough triage assessments to determine patient suitability for rehabilitation
  • Collaborate with the multidisciplinary team to develop and implement effective rehabilitation plans
  • Provide expert clinical reasoning and recommendations for patient care
  • Line manage the admissions and discharge coordinator to ensure seamless patient flow
  • Contribute to the ongoing development of clinical protocols and processes to enhance patient outcomes
  • Liaise with acute services and GPs to ensure safe patient admissions and transitions
Requirements
  • Registered Nurse, Physiotherapist, or Occupational Therapist with current NMC or HCPC registration
  • Minimum 5 years post-qualification experience in neurological rehabilitation
  • Proven expertise in conducting triage assessments and developing rehabilitation plans
  • Excellent communication and interpersonal skills
