We are seeking a highly skilled and compassionate Mental Health and Wellbeing Practitioner to join our team at HMP Stoke Heath. As a key member of our multidisciplinary team, you will work closely with service users, families, and carers to provide evidence-based interventions and plan care for adults with severe mental health problems.
Key Responsibilities- Carry out risk assessments and risk management with the multidisciplinary team to ensure the safety and well-being of service users.
- Engage with service users in highly distressing or emotional circumstances, providing calm and empathetic support.
- Develop formulations and deliver psychologically informed evidence-based interventions to a caseload of male service users.
- Work collaboratively with service users, families, and carers to develop care plans that are focused on strengths and outcomes.
- Attend multidisciplinary reviews and liaise with care coordinators to monitor progress and support service users.
- A graduate certificate or postgraduate certificate in Mental Health and Wellbeing Practitioners (Adult Specialist Mental Health).
- Experience of working with people with mental health needs gained through a graduate certificate or postgraduate certificate training with significant supervised practice.
- Experience of running groups and activities, analysing and communicating complex information verbally and in writing.
