About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our Home Treatment Team. The successful candidate will be responsible for providing intensive home treatment to individuals experiencing mental health crises, working closely with the multidisciplinary team to develop and implement individualized care plans.

Key Responsibilities

  • Provide intensive home treatment to individuals experiencing mental health crises
  • Develop and implement individualized care plans in collaboration with the multidisciplinary team
  • Work closely with service users, carers, and other professionals to ensure seamless care coordination
  • Conduct thorough assessments and risk assessments to inform care planning
  • Provide education and support to service users and carers to promote recovery and well-being

Requirements

  • Registered Mental Health Practitioner (RMN, OT, or Social Worker)
  • Extensive experience in mental health crisis intervention and home treatment
  • Strong assessment and risk management skills
  • Excellent communication and interpersonal skills
  • Ability to work flexibly and respond to changing service user needs
