Assistant Practitioner in Residential Care

We are seeking a dedicated and compassionate Assistant Practitioner to join our residential care team. As a key member of our team, you will play a crucial role in delivering high-quality care to patients with complex needs.
Key Responsibilities- Care Delivery: Deliver prescribed care to a defined group of patients, taking into account their unique needs and preferences.
- Care Planning: Assess, plan, and deliver care with patients and their families, prioritizing patient choice and dignity.
- Team Collaboration: Contribute to a multidisciplinary approach to patient care, liaising effectively with external professionals and colleagues.
- Record Keeping: Maintain accurate and up-to-date records of patient care delivered.
- Equality and Diversity: Promote and model equality and diversity principles across the team, respecting patient rights and dignity.
- Communication: Develop and maintain positive relationships with patients, colleagues, and visitors, responding promptly to requests and concerns.
- Competitive Salary: £25,000 - £30,000 per annum, depending on experience.
