The ideal candidate will have excellent communication skills and be able to work with individuals to create person-centred support plans and hold weekly keywork sessions to achieve their goals.

Key Responsibilities:

  • Run activities linked to seasonal events or celebrations to improve social isolation and specific learning linked to support
  • Manage risk and safeguarding for all residents
  • Attend multi-agency meetings and provide written reports
  • Manage void properties and allocations

Requirements:

  • Able to prioritise workload and provide non-judgmental support to customers
  • Support customers to reach their potential and identify their goals through individual support plans

Location: Durham

Rate: £29,627

Length: Fixed term until March 2025

Hours: 37 hours per week with occasional weekend/on call working
