This role is part of our Virtual Ward service, which provides hospital-standard treatment within patients' homes. We work closely with the acute trust to deliver high-quality care and prevent unnecessary hospital admissions.

As a Clinical Care Specialist, you will use your clinical skills to provide hospital-level care at home, including IV therapy, end-of-life symptom control, and wound management. You will work as part of a multi-professional team to provide holistic care to patients.

The service covers West Kent and provides nursing care within patients' homes. You will be responsible for assessment, planning, implementation, and evaluation of care, as well as developing integrated care plans with patients.

Additional Working Pattern: Long days covering a 7-day service.
