Job Title: Occupational Therapist - Reablement Specialist

About the Role:

Optalis is seeking a qualified Occupational Therapist to join our Short-Term Support and Rehabilitation Team (STS&R). As a key member of our team, you will work closely with our customers, providing six weeks of care and support to help them regain their confidence and independence in their own home.

Key Responsibilities:

  • Support customers with rehabilitation and reablement needs
  • Work collaboratively with our multi-disciplinary team, including Physiotherapists, Rehabilitation Care Assistants, and Support Staff
  • Develop and implement personalized care plans to meet the needs of our customers
  • Provide regular updates and progress reports to our team and stakeholders

Requirements:

  • Qualified Occupational Therapist with experience in rehabilitation and community care
  • Registered with the Health and Care Professions Council (HCPC)
  • Ability to drive and have access to a vehicle
  • Excellent communication and interpersonal skills
