Complex Care Coordinator

Bracknell Forest Council seeks a highly skilled Community Support Specialist to join their mental health team. This Band 6 position offers an exciting opportunity for a registered nurse, other healthcare professional, or social worker to work across the Bracknell Community Mental Health Team (CMHT) and substance misuse service, New Hope.
As a Community Support Specialist, you'll work closely with colleagues to understand the complex interplay between mental health and substance misuse. You'll use evidence-based therapeutic interventions to address co-occurring mental health, alcohol, and substance use needs. Risk and care assessments will inform care and support provision.
