Social Reintegration Professional

- To conduct comprehensive assessments of Service Users' needs and develop effective action plans.
- To work closely with Local authorities and Partners to identify and assess Service Users for the reconnection service.
- To provide advocacy support to Service Users to secure suitable accommodation in their area of local connection.
- To maintain accurate records and contribute to the development of strategies to prevent rough sleeping.
