Key Responsibilities
  • To conduct comprehensive assessments of Service Users' needs and develop effective action plans.
  • To work closely with Local authorities and Partners to identify and assess Service Users for the reconnection service.
  • To provide advocacy support to Service Users to secure suitable accommodation in their area of local connection.
  • To maintain accurate records and contribute to the development of strategies to prevent rough sleeping.
