About the Role

We are seeking a highly skilled and dedicated Complex Case Manager to join our team at Lancashire and South Cumbria NHS Foundation Trust. As a key member of our healthcare team, you will be responsible for providing expert-level advice, guidance, and support to individuals, families, and healthcare professionals.

Key Responsibilities:

  • Develop and implement comprehensive care plans tailored to the unique needs of complex patients.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Provide education and training to patients, families, and healthcare professionals on complex care management.
  • Conduct thorough assessments and evaluations to identify patient needs and develop strategies for optimal care.

Requirements:

  • HCPC registration as a healthcare professional.
  • Proven experience in complex care management or a related field.
  • Expert-level knowledge of healthcare policies, procedures, and regulations.
  • Excellent communication, interpersonal, and problem-solving skills.
