Job Title: Adult Social Care Community Support Worker

About the Role

We are seeking a highly skilled and experienced Adult Social Care Community Support Worker to join our team. As a key member of our care team, you will play a crucial role in providing high-quality care and support to individuals in their own homes and communities.

Key Responsibilities

  1. Conduct thorough assessments of individuals' needs, including risk assessments and care planning, to ensure tailored support.
  2. Support the safe administration of medication and monitor individuals' health needs.
  3. Lead and manage a team of care workers, providing guidance and support to ensure effective team communication and performance.
  4. Monitor and record care delivery, identifying areas for improvement and implementing changes as needed.
  5. Support individuals with daily activities, such as bathing, dressing, and mobility, promoting independence and dignity.
  6. Encourage participation in activities and foster a positive living environment.
  7. Maintain a safe and clean environment, adhering to health and safety regulations.
  8. Take responsibility for delegated areas of work, including specific projects, and contribute to service development.
  9. Participate in or lead colleague meetings, undertake training, and develop individual and group programs to meet specific objectives.
  10. Represent the Registered Manager/Deputy Manager at meetings and contribute to the development of services in accordance with directorate policy.
  11. Maintain links with partner organizations, the voluntary and private sector, as appropriate.
  12. Work within the Gold Standards Framework.

Person Specification

Essential Requirements

  1. Previous experience in a similar role within a care home or similar environment.
  2. Appropriate qualification, NVQ level 3 in Health and Social Care (or equivalent) or above.
  3. Significant knowledge and application of relevant legislation, e.g., Care Standards Act 2000, Community Care Act 1991.
  4. Significant knowledge of risk assessment process.
  5. Significant knowledge of care planning process obtained in a similar environment.
  6. Ability to lead, manage, and communicate effectively with team members.
  7. Ability to plan, organize, and monitor own work.
  8. Current medication training and experience.
  9. Experience of understanding and applying anti-discriminatory practice.
  10. Management of colleagues, including use of people management policies and procedures.
  11. Understanding of Safeguarding Adults policy and procedures.
  12. Significant experience of assessing individual client needs.

  1. Able to communicate effectively, both orally and in writing, with individuals, colleagues, relatives, other professionals, and senior managers.
  2. Strong ability to work flexibly, managing changing and competing priorities.
  3. Able to provide effective line management to achieve objectives.
  4. Able to contribute to service development.
  5. Good IT skills.
