Job Summary

The Clinical Care Specialist plays a pivotal role in enhancing the level of care provided to patients through advanced practice interventions. This role involves autonomously managing complete episodes of clinical care across all age groups and demographics, encompassing a wide range of clinical conditions, diagnosing, treating, and referring patients as appropriate.

The Clinical Care Specialist operates within a multi-disciplinary team and is instrumental in mentoring and supporting both nursing staff and trainee practitioners.

Main Duties of the Job

Clinical Responsibilities:

  • Patient Care: Deliver advanced and complex clinical care, providing assessment, diagnosis, treatment, and prescribing for patients with acute, chronic, and complex conditions.
  • Autonomous Practice: Work independently within the scope of practice to manage complete clinical episodes, using advanced diagnostic skills and clinical judgment.
  • Case Management: Manage patient cases from initial assessment to conclusion, including follow-ups and referrals, ensuring continuity of care.
  • Prescribing: Utilise prescriptive authority responsibly following current guidelines and protocols to enhance therapeutic outcomes for patients.
  • Home Visiting: Undertake home visiting as and when required, including triage of home visits.

Leadership and Guidance:

  • Mentorship: Provide mentorship and supervision to nursing staff, trainee advanced practitioners, and other healthcare professionals to enhance team competencies.
  • Service Development: Lead the development and implementation of new clinical services and pathways, improving processes and patient care.
  • Professional Development: Engage in continuous professional development to keep abreast of medical advancements and to further develop clinical skills.

Research and Quality Improvement:

  • Research Participation: Participate in clinical audits and research activities to contribute to the evidence base for practice.
  • Quality Assurance: Implement quality improvement initiatives and ensure compliance with healthcare standards and best practices in patient care.

The Fuller and Forbes Partnership is a primary care provider with practices currently in Gateshead, Plymouth, Exeter, Barnstaple, Leeds, and Burnley. The 4 sites in Gateshead operate at Crawcrook, Blaydon, Grange Road, and Rowlands Gill, providing care to over 20,500 patients. Administrative and Organisational Duties:

  • Record Keeping: Maintain comprehensive and accurate patient records using SystmOne.
  • Meetings: Attend and contribute to team meetings, providing updates and insights on patient care strategies and outcomes.

Person Specification:

Qualifications:

  • Essential:
  • Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC) or other regulatory body as appropriate.

Experience:

  • Essential:
  • Clinical Skills: Extensive experience in managing complete episodes of clinical care.
  • Advanced clinical skills in a primary care setting with proficiency in using clinical diagnostic tools and conducting complex clinical procedures.

Leadership and Mentorship:

  • Experience in a leadership role.
  • Demonstrated ability to lead service improvement initiatives and develop new clinical pathways.

Regulatory Knowledge:

In-depth knowledge of current national health policies and clinical guidelines relevant to primary care and advanced practice.

Professional Development:

Engage in continuous professional development to keep abreast of medical advancements and to further develop clinical skills.
