Job summary
The Advanced Clinical Practitioner (ACP) playsa pivotal role in enhancing the level of care provided to patients throughadvanced practice interventions. This role involves autonomously managingcomplete episodes of clinical care across all age groups and all demographics,encompassing a wide range of clinical conditions, diagnosing, treating, andreferring patients as appropriate. The ACP operates within a multi-disciplinaryteam and is instrumental in mentoring and supporting both nursing staff andtrainee practitioners.
Key responsibilities include the applicationof advanced clinical skills to assess, diagnose, treat, and advise patientsacross a variety of settings including face to face, telephone video andeconsultations, ensuring the use of evidence-based practice. The ACP will alsolead in the development of new services and pathways to improve the quality ofcare and patient outcomes. This role requires dynamic clinical leadership andactive participation in practice research activities to promote a culture oflearning and continuous improvement within the practice.

ProfessionalDevelopment: Engage in continuous professional development to keep abreast ofmedical advancements and to further develop clinical skills.
Research and QualityImprovement:
ResearchParticipation: Participate in clinical audits and research activities tocontribute to the evidence base for practice.

Qualifications
Essential
Educational Background: Masters degree in advanced Practice, or equivalent post-graduate qualification, or full accreditation or eportfolio. Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC) or other regulatory body as appropriate.
Experience

Clinical Skills: Extensive experience in managing complete episodes of clinical care Advanced clinical skills in a primary care setting with proficiency in using clinical diagnostic tools and conducting complex clinical procedures. Communication and Interpersonal Skills: Excellent communication skills The ability to convey complex information to patients and team members effectively. Strong interpersonal skills with the capability of working collaboratively within a multidisciplinary team.
