Young People's Pathway Planner

Rotherham Metropolitan Borough Council is seeking a dedicated Young People's Pathway Planner to join their team. This full-time position comes with a salary of £29,000 - £34,000 per annum.
The successful candidate will have a strong understanding of the needs of young people leaving care, including knowledge of the Pathway Planning process. With excellent communication skills and the ability to form positive relationships, they will be able to work collaboratively with young people, families, and other agencies to ensure effective support.
Responsibilities include holding a caseload of young people aged 16-25, providing practical and financial support to help them achieve their goals. This may involve advocacy, negotiation, and collaborative work with colleagues and external agencies to identify and address barriers to success.
A commitment to promoting equal opportunities and anti-oppressive practice is essential, as well as the ability to work under pressure to meet deadlines. A high standard of assessment and planning skills is required, prioritizing the voice of the young person in their Pathway Plans.
This role requires the successful applicant to undergo a disclosure of criminal records check at an enhanced level.
