Job Title: Adult Social Care Support Specialist

About the Role:

We are seeking a skilled and dedicated Adult Social Care Support Specialist to join our team at Hatched Recruitment Group. As an integral part of our social care services, you will play a vital role in supporting vulnerable adults to achieve their full potential.

Key Responsibilities:

Social Work and Case Management

  • Work within a strengths-based model of practice to support residents improve their independence and wellbeing.
  • Undertake assessments, reviews, and support planning to enable residents to maximize their independence and quality of life.
  • Provide practical support to residents and carers, including accessing living aids and welfare benefits.

Collaboration and Communication

  • Liaise with colleagues across the Haringey health and social care partnership to gather information relevant to assessment and support planning activities.
  • Contribute to effective Multi-Disciplinary Team working by inputting through meetings and working with others to achieve best outcomes for service users.

Administrative and Record-Keeping Tasks

  • Maintain and update reliable, timely, and accurate case notes, systems, and other records.
  • Support social workers in collecting information and presenting to legal proceedings as required.

Requirements:

Essential Qualifications and Skills:

  • Awareness and understanding of the part that Adult Social Care plays in maximizing the independence of vulnerable adults.
  • Demonstrably gained experience or exposure to social care in a voluntary, paid, or personal basis.
  • Evidence of researching and understanding social care ethics and values.
  • Ability to develop positive and productive relationships with people, including empathy, motivation, and inspiration.
  • Enhanced DBS check.

Salary Information:

The estimated salary for this role is £23 per hour.
