Community Nurse

We are seeking a highly skilled and dedicated Community Nurse to join our team at East Coast Community Healthcare CIC. As a Community Nurse, you will be responsible for providing high-quality, person-centered care to adults in their own homes, residential care, or clinic settings.
Key Responsibilities- Conduct holistic assessments for patients referred to the service, including environmental, moving and handling assessments, pressure area risk, and nutritional needs.
- Participate in the delivery of nursing interventions, such as urinary catheterization, administration of medication via injection, wound care, and palliation and end-of-life care.
- Support patients in self-care using health coaching techniques and work collaboratively with residential care home colleagues to promote high standards of care.
- Contribute to the clinical governance agenda through participation in clinical risk assessment and clinical audit.
- Work as part of the multidisciplinary Primary Care Home team to deliver planned and unplanned services, and contribute to the development of care pathways for patients requiring a range of healthcare needs.
- Professional Registration with the Nursing and Midwifery Council (NMC) as a Registered Nurse (Adult).
- BSc in Nursing Practice or equivalent.
- Excellent communication and interpersonal skills, with the ability to work unsupervised and demonstrate clinical knowledge and competence within the scope of clinical practice.
- Previous experience of working in healthcare, with a willingness to work across different sites and travel to alternative sites and across the community as required.
