About the Role

We are seeking a highly skilled and dedicated Community Nurse to join our team at East Coast Community Healthcare CIC. As a Community Nurse, you will play a vital role in providing high-quality care to patients in their own homes, clinics, or residential settings.

Key Responsibilities
  • Conduct holistic assessments for patients referred to the service, identifying their needs and developing personalized care plans.
  • Deliver nursing interventions, including wound care, medication administration, and palliative care, to support patients in maintaining their health and independence.
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery and effective communication with patients, families, and healthcare professionals.
  • Participate in clinical supervision and education to maintain and enhance clinical skills and knowledge.
  • Contribute to the development of care pathways and clinical guidelines to ensure best practice and high-quality care.
Requirements
  • Professional registration with the Nursing and Midwifery Council (NMC) as a Registered Nurse (Adult).
  • BSc in Nursing Practice or equivalent.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Competent IT skills and ability to maintain accurate and contemporaneous patient records.
  • Ability to work unsupervised and demonstrate clinical knowledge and competence within the scope of clinical practice.
