Key Responsibilities- Develop and implement effective care plans to support adults in living independently.
- Collaborate with other professionals to ensure a multi-agency approach that meets the unique needs of each individual.
- Represent service users' views at meetings and exchange information, make decisions, and review progress to achieve optimal outcomes.
- Build relationships with local agencies and communities to provide timely signposting and referrals when necessary.
- Identify potential deprivation of liberty and work towards ensuring the least restrictive means of care provision prior to legal proceedings.
- Maintain accurate records according to the Adult Services' Recording Policy to ensure transparency and accountability.
- Participate in reflective practice and supervision to continuously improve skills and knowledge.
- Stay updated on legislative changes and meet continuing professional development requirements to maintain expertise.
- A social work qualification or equivalent is required.
- Registration with the relevant health professional council/governing body (Social Care England) is mandatory.
