About the role
We are seeking an experienced and skilled Advanced Community Matron to join our Neighbourhood Nursing team.
Job overview
The successful candidate will be responsible for providing advanced nursing care to complex patients in their own homes and community settings.
As a key member of the team, you will work closely with Neighbourhood Nurses to deliver high-quality care, support, and coordination of services.
Main duties and responsibilities
- Conduct comprehensive assessments of patients' needs and develop individualized care plans.
- Provide advanced nursing care, including disease management, palliation, and end-of-life care.
- Work closely with multidisciplinary teams to coordinate care and services.
- Lead and facilitate community multidisciplinary team meetings and case conferences.
- Participate in clinical audit, benchmarking, and evaluation of services.
- Develop and implement policies, procedures, and guidelines relevant to complex care.
- Provide clinical supervision and education to nursing staff.
- Collaborate with other healthcare professionals to ensure seamless care delivery.
Requirements
We are looking for an experienced and skilled individual with:
- Substantial experience working as a senior practitioner in a community nursing team.
- Excellent advanced assessment skills and ability to demonstrate evidence-based care.
- Ability to work autonomously and lead successful change management.
- Excellent interpersonal and negotiation skills.
- Nurse formulary prescriber and independent prescribing module completion or willingness to undertake.
- Education to 1st degree level or equivalent and a teaching certificate (ENB 998/997).
