Job Summary

The Discharge Matron will provide visible and accessible leadership, focusing on ensuring the efficient and timely discharge of patients from the hospital, while maintaining high standards of care throughout the discharge process. This role ensures smooth patient flow through the hospital, with a particular focus on reducing delays and enhancing communication between clinical teams and external care providers.

The Discharge Matron will ensure that patients are treated with compassion, dignity, and respect, valuing the diversity of the local community and wider population. They will also promote best practices in line with Trust policy and national guidelines.

The Discharge Matron will collaborate with the Divisional Directors, General Manager, and other key stakeholders to ensure that discharge processes are aligned with both the operational needs of the hospital and patient-centred care.

Main Responsibilities
  • Lead on and take appropriate action in cases of staff discipline, grievance, poor performance, and sickness that impact the discharge process.
  • Manage the quality of the discharge process across the Trust.
  • Liaise with the PALS regarding informal complaints, proactively resolving issues to prevent escalation.
  • Support the development and implementation of tailored education and training programs focused on discharge procedures for nursing staff.

Qualifications
Essential
  • Registered nurse qualification
  • Degree or equivalent clinical/advanced practice
  • Minimum 5 years post registration
  • Masters Degree or working towards
Desirable
  • Evidence of post registration/ qualification
  • Management/Leadership qualification
Experience
Essential
  • Clinical expertise relevant to the post
  • Experience of managing staff
  • Minimum of 3 years' experience working at Band 7 or equivalent in an acute setting.
  • Leadership of clinical governance and patient safety
  • Achievement of high quality patient care through evidence based practice
  • Management of finances within a budget, including delivery of cost improvements
  • Management/Clinical Leadership experience
  • Experience of successfully leading and implementing change
  • Experience in analysing complex problems and implementing workable solutions
  • Ability to think and plan strategically, operationally and creatively and to prioritise work programs in the face of competing demands

  • Evidence of post registration/ qualification
