Job Summary:

We are seeking an experienced Social Worker to join our dynamic Adult Community team. As a crucial member of the team, you will work collaboratively with individuals, families, and caregivers to conduct Care Act 2014 assessments, evaluate needs, and manage both short-term and long-term cases.

Main Responsibilities:

  • Undertake complex person-centred assessments with customers and plan for the provision of support and/or services to assist them meet their outcomes.
  • Support carers in complex situations and ensure accurate recording of information onto social care records.
  • Contribute to local knowledge about universally available services and signpost/advocate for customers accordingly.
  • Practice in line with legal requirements, demonstrating a detailed understanding of the Care Act, Mental Capacity Act, and Continuing Health Care guidance.
  • Identify where adults may be at risk and act in accordance with safeguarding policies and procedures.

About the Role:

This position comes with a market supplement payment, subject to review annually. You will be part of a friendly and welcoming team at Wiltshire Council, committed to professional development and career growth.

Requirements:

  • Registration with Social Work England is essential.
  • A solid understanding of the Care Act 2014 is required.
  • You must approach risks positively and be passionate about making a difference.

Working Arrangements:

The role involves flexible working, with occasional requirement to work outside standard hours, including weekends, to meet service needs.

Estimated Salary: £40,000 - £50,000 per annum
