Salary Range: Up to $65,000 per year (based on experience)
Job Description: The Case Manager - Pawtucket Health Home is responsible for coordinating services for consumers, minimizing stressors, and facilitating the reduction of psychiatric and substance abuse disorder symptoms.
Key Responsibilities:
- Service Coordination: Develop and implement individualized service plans to meet the unique needs of each consumer.
- Support and Guidance: Provide empathetic and non-judgmental support to consumers, promoting their autonomy and dignity.
- Crisis Intervention: Conduct thorough assessments to identify risk factors and provide crisis interventions as needed.
- Skills Training: Educate consumers on essential life skills, empowering them to maintain independence.
- Community Resources: Connect consumers with community resources and advocate on their behalf when necessary.
- Communication: Communicate effectively with landlords, courts, families, treatment teams, and consumers themselves.
- Recordkeeping: Accurately document consumer interactions and progress, adhering to Health Information Services and center requirements.
- Relationships: Foster positive relationships with Center staff, consumers, community agencies, and the public.
- Professional Development: Stay up-to-date on industry best practices and emerging trends by attending relevant trainings and conferences.
Requirements:
- Degree Requirement: Possess a Bachelor's degree from an accredited four-year College or University in Human Services or a related field.
- Experience: Preferably have experience in the Human Services field, demonstrating a strong understanding of case management principles and practices.
- Driver's License: Hold a valid driver's license and possess insured auto coverage.
- Flexibility: Demonstrate flexibility in adapting to changing situations and priorities.
