Pawtucket Health Home Job OverviewWe are seeking a skilled and compassionate Health Home Care Specialist to join our team at Pawtucket Health Home. This role is ideal for individuals who are passionate about providing supportive services to consumers with complex needs.
About the Position:
- Coordinate services for consumers to promote independence and well-being.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans.
- Provide education and training to consumers on skills such as medication management, healthy habits, and coping mechanisms.
Key Responsibilities:
- Conduct comprehensive assessments to identify consumer needs and preferences.
- Develop and implement individualized care plans that address physical, emotional, and social needs.
- Provide ongoing support and coaching to consumers, including crisis intervention and risk management as needed.
- Maintain accurate records and reports, ensuring confidentiality and compliance with regulatory requirements.
- Collaborate with community resources and providers to ensure seamless delivery of services.
Requirements:
- Bachelor's degree from an accredited four-year College or University in Human Services or related field.
- Experience in healthcare or human services field preferred.
- Valid driver's license and insured auto required.
- Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.
