About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our Primary Care Network as a Band 6 equivalent. The successful candidate will be a qualified Nurse or Allied Health Professional with a strong background in mental health and a passion for delivering high-quality care.

The role will involve working closely with our primary care team to assess and manage the mental health needs of our patients. You will be responsible for providing clinical expertise and support to GPs and other healthcare professionals, as well as developing and implementing care plans to meet the individual needs of our patients.

We are looking for someone who is proactive, innovative, and committed to delivering excellent patient care. If you have a strong clinical background, excellent communication skills, and a passion for mental health, we would love to hear from you.

Key Responsibilities
  • Assess and manage the mental health needs of patients in primary care
  • Provide clinical expertise and support to GPs and other healthcare professionals
  • Develop and implement care plans to meet the individual needs of patients
  • Work closely with the primary care team to ensure seamless care delivery
  • Contribute to the development of mental health services and initiatives within the Primary Care Network
Requirements
  • Registered Nurse or Allied Health Professional with a strong background in mental health
  • Experience of working in primary care or a similar setting
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Strong clinical skills and knowledge of mental health
