About the Role

We are seeking a skilled and compassionate Community Nurse to join our team in the beautiful North Kerrier area. As a Community Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their own homes.

Key Responsibilities
  • Assess, plan, implement, and evaluate care for patients, carers, and relatives in their home environment.
  • Work autonomously, providing effective, evidence-based care in line with competency and training records, organisational policies, and procedures.
  • Collaborate with GP's, health and social care colleagues, including therapists and voluntary organisations, to deliver comprehensive care.
  • Travel independently across the West Locality to support other teams and fulfill the requirements of the post.

Requirements
  • Registered Nurse (Adult)
  • Evidence of recent relevant professional development
  • Current Professional Registration - NMC
  • Ability to provide skilled, evidence-based nursing care to a wide range of patients
  • IT skills to include use of Word, Excel, email, internet, recording systems
