Diabetes Specialist Nurse

We are seeking a highly skilled and experienced Band 6 Diabetes Specialist Nurse to join our team at Norfolk Community Health and Care NHS Trust. The successful candidate will have a strong background in diabetes management and be able to provide expert care and support to patients with complex diabetes needs.
About the RoleThe Diabetes Specialist Service is a busy nurse-led team that works across West Norfolk to assess and manage people with complex diabetes and other co-morbidities. We offer support and education to other healthcare professionals and run structured education sessions for patients.
The successful candidate will be responsible for managing a caseload of patients with complex diabetes needs, using evidence-based practice and patient-centered goals to assess, plan, implement, and evaluate interventions. They will also be required to work independently in patients' homes across the county with access to the diabetes team remotely.
Key Responsibilities- Manage a caseload of patients with complex diabetes needs
- Provide expert care and support to patients with diabetes
- Work independently in patients' homes across the county
- Collaborate with other healthcare professionals to provide comprehensive care
- Develop and implement patient-centered care plans
- Registration in adult nursing with extensive post-registration experience
- Recent clinical experience in management of individuals with diabetes
- Good knowledge of relevant NHS policy, most notably NSF for Diabetes and NICE
