Job Title: Band 6 Diabetes Specialist Nurse

We are seeking a highly skilled and experienced Band 6 Diabetes Specialist Nurse to join our community diabetes team in West Norfolk. As a key member of our team, you will play a vital role in providing expert care and support to individuals with diabetes in a variety of settings.

About the Role

The successful candidate will have extensive post-registration experience in the management of individuals with diabetes and a strong understanding of relevant NHS policy, including the NSF for Diabetes and NICE guidelines. You will be responsible for managing a caseload of patients with complex diabetes needs, using evidence-based practice and patient-centered goals to assess, plan, implement, and evaluate interventions.

Key Responsibilities
  • Provide expert care and support to individuals with diabetes in community settings, including home visits and telephone support.
  • Work collaboratively with primary health care teams to provide holistic care and support to patients with diabetes.
  • Develop and implement individualized care plans to meet the unique needs of patients with diabetes.
  • Provide education and support to patients, their families, and carers to enable them to self-manage their diabetes and reduce the risk of complications.
  • Collaborate with other healthcare professionals to provide comprehensive care and support to patients with diabetes.
Person Specification
  • Registration in adult nursing with extensive post-registration experience in the management of individuals with diabetes.
  • Good knowledge of relevant NHS policy, including the NSF for Diabetes and NICE guidelines.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
