Job Title: Band 6 Diabetes Specialist Nurse

A challenging and rewarding opportunity has arisen within the Diabetes Specialist Services at Norfolk Community Health and Care NHS Trust at Band 6 level. We are seeking a skilled and experienced Diabetes Specialist Nurse to join our team in North Norfolk.

About the Role

The successful candidate will be responsible for managing a caseload of patients with complex diabetes needs, using evidence-based practice and patient-centered goals to assess, plan, implement, and evaluate interventions. The role will involve health education to service users, their families, carers, and other agencies involved in their care management.

Key Responsibilities
  • Work within the Diabetes Team providing expert knowledge and skills.
  • Manage a caseload of patients with complex diabetes needs.
  • Provide health education to service users, their families, carers, and other agencies.
  • Work independently, visiting patients in their homes across North Norfolk.
  • Provide educational support to professionals running Diabetes clinics in general practice.
  • Deliver a specialist Diabetes Service to community care, clinics, nursing/care homes, and housebound patients.
Person Specification

The ideal candidate will have:

  • Registration in adult nursing with extensive post-registration experience.
  • Recent clinical experience in the management of individuals with diabetes.
  • Excellent interpersonal skills and ability to work effectively within teams.
  • Good knowledge of relevant NHS policy, including the NSF for Diabetes and NICE.
