Your goal will be to meet their physical, emotional, and social needs according to their support plans, helping them achieve their goals and aspirations as independently as possible. You will also use technology to give the people we support more choice and control over their lives.
Key Responsibilities- Provide person-centered support to individuals with varying care needs
- Respond to individuals' needs and provide safe, positive support in all aspects of daily living
- Work collaboratively with multidisciplinary teams to ensure high-quality care and support
- Use technology to give individuals more choice and control over their lives
- Full UK driving license
- Access to a motor car
- Level 2 Diploma in Care or relevant experience
- Experience working in a health or social care setting
- Supporting individuals with behaviors that require intensive support planning and management
- Collaborating with multidisciplinary teams and participating in complex case reviews
- Working independently and as part of a team
