Key Responsibilities- Assess and manage a caseload of clients on the reablement unit within set timeframes.
- Plan, deliver, and evaluate treatment needs of clients with complex social and health care needs.
- Participate in developing and delivering high-quality services to the local population.
- Promote effective teamwork within the trust and delegate activities appropriately.
